Before submitting a cancellation request:

Step 2: Review cancellation requirement
Before completing your request, you are responsible for the following:
14-Day Notice Required:
Cancellation requests must be submitted at least 14 days before your next billing date to avoid being charged for your next billing cycle.

Step 3: Submit the cancellation form
You may submit the cancellation form at any time. However, to avoid being charged for your next billing cycle, your request must be submitted at least 14 days before your next payment date.
By submitting this form, you are acknowledging that you have reviewed and fulfilled the cancellation requirement, including:
